How to register?

To register at a territorial polyclinic, you need to:

• visit the nearest local polyclinic,
• submit the required documents,
• sign an agreement with the polyclinic to become a registered beneficiary.

Requirements

To register at a polyclinic, you must:

• have Armenian citizenship or permanent/special residency status in Armenia,
• be registered at a residential address in Armenia.

Required documents

To register, you must provide the following documents at the polyclinic:

• passport,
• permanent residence card or special passport,
• public Services Number (Social Card Number).

Delivery period and procedure

The registration is completed on the same day after submitting the required documents.
