Assess the level of disability

In Armenia, a person's functional assessment is carried out through the "e-disability" electronic system. The system is operated by the Unified Social Service of the Ministry of Labor and Social Affairs of Armenia.

You can apply for this service in person by booking an appointment in advance.

How to obtain a referral for functional assessment?

To obtain a referral, the person with disability or their legal representative should:

  • visit a medical care and service provider organization,
  • receive a referral for functional assessment from the doctor (the doctor issues the referral after completing the necessary diagnostic, medical, and rehabilitation measures).

The doctor fills in all required personal and medical information in the referral. You can view the referral through the ARMED system. If the applicant's email is registered in the system, the referral can also be sent to their email.


The referral for the functional assessment is based on clinical data and results from laboratory and instrumental diagnostic tests.
There is also a list of minimum necessary information, depending on health condition groups, which must be included in the referral while applying.

Once you obtain the referral, follow these steps after clicking the "Continue" button:

  • visit e-disability.am (to view the English version use the browser's translation tool), 
  • click on the "Application" button,
  • choose one of the 4 application types: primary assessment, repeated assessment, re-assessment, or determination of cause of death (in this case, primary assessment if it is the first application),
  • select the subtype of the application,
  • choose the reason for applying,
  • indicate who is submitting the application, 
  • fill in the personal information and the self-assessment form (the information must be filled in exclusively in Armenian),
  • follow the system's instructions.

The applicant will be notified about the next steps of the application through the chosen notification method (in-person, by post, or via email).

Attention:

  • The applicant's personal data remains confidential to the committee members and all participating specialists until the decision is made.
  • For applicants under 16 years old, the application must be completed by the legal representative of the person.

Requirements 

The presented documents must be valid within the last 30 days. Otherwise, after verifying the application, the system will notify that the submitted documents are incomplete.


Required documents 

To apply for functional assessment, it is required to have a Public Services Number (PSN, social security card) or a certificate stating the lack of a Public Services Number.


Delivery period and procedure 

  • The application is sent to the summarizing specialist.
  • The summarizing specialist checks the authenticity of the attached documents and selects the specialists needed for the person's assessment.
  • A committee is automatically formed in the system to assess the decision based on the functional assessment.
  • The committee includes doctors, psychologists, occupational therapists, speech therapists, and other specialists as necessary.
  • Committee members are selected randomly from the registry of specialists involved in the functional assessment through the electronic system.
  • After this, the application is processed, and the status is assigned within 3 working days.
